Please note we can only accept requests to register from those who have no current GP in the area.

Procedure for Coalisland Practices:

Should you wish to register within the Coalisland area, you should contact any practice to request registration. The practice will take your details, name, address, etc and add it to a 'Patient Registration Request' list. This list will be forwarded to the local Health Board for allocation to a practice in the Coalisland area. The practice you have been allocated to will then contact you to advise you how to proceed with the registration.

All patients registering with the practice must provide proof of identity.  We require 2 forms of identity - (i) Photographic ID (ii) Proof of address. Unfortunately we will be unable to process your registration until we are in full receipt of the information requested. Please ensure you bring this information with you when leaving in your registration form.

All patients must attend the Practice Nurse for a pre-registration health check.  An appointment will be scheduled for you.  Please contact reception for further information, a member of our reception team will be happy to answer any questions you may have regarding registration. 

The surgery is on the ground floor allowing easy access for disabled patients.
